Pure Heart Home Care (PHHC) services allow clients to stay in their home and “age in place” while receiving much needed care and companionship. Our process for beginning services to clients starts with a conversation.
Senior Care Services
  • PHHC talks with the family and reviews the client’s present needs. Details regarding eating habits, ambulation, and client activities are included.
  • When it is advisable, a consultation with the physician(s) of the care recipient, social workers, hospitals, or nursing home staff also take place.
  • From the initial discussions, referrals for Medicare, Medicaid, and Long Term Care Insurance can be offered to the client’s family, as needed.

Recommendations regarding in-home care can also result from these initial discussions.

If PHHC is engaged in service to the client, an individualized plan is developed in conjunction with the family, to manage daily needs. Using the PHHC Matching System, client needs are matched with the most appropriate caregiver in the database, and the process moves forward. This intensive approach provides the most satisfaction for clients and their families, as individual personality, skills, and physical requirements are all considered in the caregiver selection. In addition, a large database of potential caregivers provides a wide variety of opportunity for selecting the best caregiver to most effectively meet client needs.

PHHC screens and selects all caregivers for clients to ensure proper verification, certification, training, background checks, reference checks, yearly physicals and TB test are all checked before implementation of care. Once our PHHC caregiver is selected, trained and placed in your home PHHC continues to monitor the caregiver and quality of care to ensure a successful transition. Telephone calls, check-in, and home visits are a part of the ongoing quality control at PHHC.

 
 

PHHC Live-In Care Program

 

Our PHHC Live-In Care Program iis a cost-effective arrangement that provides continuous home care for clients at a reduced cost. Live-In Caregivers reside with the client to service all the clients’ home care needs on a daily basis. Our Caregivers adjust to the client’s schedule and provide all non-medical home care needs in order to provide the highest level of satisfaction to the client as well as peace of mind for the family.

Live-Out Hourly Care

Our Live-Out Hourly PHHC Caregiver will come to the senior's residence, hospital, nursing home or assisted living facility and assist her or him according to the personalized care plan and time frame agreed upon.

  • Service is billed by the hour as needed or requested.
  • Our professional caregivers provide customized services to suit the client’s needs
  • Some clients require light assistance once, twice or several times a week
  • Clients may request an occasional 24-hour Caregiver
  • The minimum amount of time is four hours per visit

Whatever your needs are, we are dedicated to providing quality care at a reasonable rate.

Respite Care

Respite is requested time off for caregivers. This provides our professionals with a much-needed break in order to take care of personal business and avoid physical and emotional exhaustion. Respite is also a time for family members, caring for a loved one, to get a good night's sleep, a weekend away, or a week of vacation and to recharge.

What to Expect from PHHC Live-In Caregiver

  • To have at least 6 months care experience or more.
  • To be fully trained as a Home Health Aide or Certified Nursing Assistant
  • To have an extensive background and reference check.
  • To always provide a professional and confidential service.
  • Live-in Caregiver will stay an average of 4 to 12 weeks to build relationship with family members before taking time off for relief.
  • Assistance with hygiene, bathing, and dressing; home-safety supervision; medication reminders; meal preparation; laundry; errands; and vacuuming and routine household cleaning.
  • To undertake any other tasks that are appropriate and have been identified and agreed during the initial visit or assessment.
